- Consult
on long term care insurance, retirement communities, assisted
living and convalescent home facilities.
- Plan
for and apply for Medicaid (Title XIX) benefits, and Fair
Hearing appeals.
- Assist
in phasing in the next level of care/living. Coordinate the
move, arrange for handling and dispersal of cherished possessions,
antiques and collectibles; prepare home for sale; and help
the dependent elderly client or appropriate family member
secure the next level of care.
- Represent
the client's interests. Liaison with attorneys, doctors, etc.
Work as an advocate, protector and/or surrogate for the client.
- Mediate
problems between the client and various levels of Health Care
facilities and family. Act as liaison between administration
of Health Care facilities, the client, family and doctor.
- Serve
as Guardian or Conservator for the client with court approval.
- Serve
as a surrogate decision maker when needed.
- Process
medical claims and appeals.
